Now that you've had your intake phone call with one of our clinicians, you are officially signed up to attend our Five-Day Program. INTAKE CHECKLIST.

1) PLEASE COMPLETE THE INTAKE PACKET PRIOR TO YOUR ARRIVAL.

Please take some time to complete the intake packet that you will find below. The questionnaires included in the intake packet are designed to give us an idea of what you are dealing with, where you would like to be, and how we can better individualize our approach to your needs. Please do not stress about the questions. You do not have to go into detail about anything you're not ready to explore fully. However, honest and open answers will help us and will be very helpful for any workers compensation, retirement, disability, or other claim you may be working on.

THERE ARE A FEW WAYS YOU CAN GET YOUR INTAKE PACKET TO US:

  • Download the document and fill it out electronically, if you have that capability, and email it to us.

  • Print the document, fill it out manually, and then scan it back into your computer so that you can email it to us.

  • Print the document, fill it out manually, and bring it with you on Monday morning when you arrive. You can give it to any other of the staff members and we will get it to your clinician for you.

  • If all of this is too complicated, please make a request to your clinician that we mail you the intake packet. Fill it out and bring it with you on Monday morning when you arrive.

2) PLEASE PROVIDE US WITH YOUR HEALTH INSURANCE INFORMATION.

We bill your health insurance carrier when you attend our Five-Day Program. Please take pictures of the front and back of your health insurance card and email them to our Office Manager Lila Cancellieri at lilac@frontlinefoundations.org. You can also contact her with any questions you might have.

3) OPTIONAL ORIENTATION ZOOM MEETING.

Our Peer Counselor & Alumni Coordinator Joe Gorman hosts an optional Orientation Zoom Meeting before the start of the Five-Day Program. Although this meeting is optional, we encourage you to attend so that you can get to know Joe and the other people that you will be attending the program with. This is also a time to ask questions and learn a little bit more about the program before you get here on Monday. The Zoom meeting is held on the Wednesday ten days prior to your arrival at 6:30pm. Joe will email you a Zoom link for the meeting. Please attend if you are able, and feel free to touch base with him in the meantime if you have any questions:

DIRECTIONS.

FRONT LINE FOUNDATIONS AT THE ROCKINGHAM HEALTH CENTER.

OUR PROGRAM IS LOCATED AT THE ROCKINGHAM HEALTH CENTER IN BELLOWS FALL, VERMONT:

Park in the front parking lot at the main entrance of the Health Center and come in through the automatic sliding doors. Walk through the waiting room and make a left past the "registration" windows (those are reception for the North Star Health services co-located in our building). At the end of the hallway, turn left. You will approach another waiting room area. Turn right and you will see our sign above our glass door. Our door opens at 8:00am. You are welcome to come in for coffee and donuts and to get settled before programming starts at 8:30am.

PROGRAM LOGISTICS.

OUR DAILY SCHEDULE.

Our door opens at 8:00am each morning. You are welcome to come in for coffee and donuts and to get settled before programming starts at 8:30am. Each day, our schedule is the same, except for on Wednesdays when Recreation Therapy replaces the three morning groups. We wrap up every day at 3:00pm. To find out more about each of our daily groups, you can read the group descriptions HERE.

RECREATION THERAPY is a chance for us to spend some time in nature together. Some of our favorite activities include taking a leisurely walk on one of the local trails in our area, or visiting the property of our favorite local wildlife rehabilitator Fred Homer to see the owls and hawks that he is restoring to health. We advise you to bring some footwear that you wouldn't mind getting a little dirty in the case that we have some light rain, snow, or muddy conditions depending on the season.

The rest of our programming takes place inside of our office space. We have intentionally created an atmosphere that is relaxed and casual, as we want everyone to feel comfortable while they are here. We have coffee/tea, snacks, water, and donuts/pastries in the kitchen in between groups. Feel free to wear comfortable clothes, and even bring slippers to wear during the indoor groups if you would like.

LODGING.

Although we encourage our clients to take advantage of the shared housing that we coordinate for each group that attends our Five-Day Program, you are under no obligation to participate in shared lodging. We understand that many of you are local and may choose to commute, or may have a variety of other reasons to return home each day. Even so, you are welcome to join your peers at the house for dinner, or to visit after the day of programming ends at 3:00pm.

Our Peer Counselor & Alumni Coordinator Joe Gorman is in charge of coordinating lodging for all those who wish to participate in this element of the program. We find that there is an enhanced sense of camaraderie and a powerful peer support element that comes from staying together under the same roof and eating meals together. We select from a list of local Airbnbs depending on the size of the cohort (the group of people attending the program together), so that everyone has their own bedroom in the house.

NOTE: If there is not enough demand for shared housing within a cohort and you need local lodging for yourself, Joe will provide with a list of recommended places to stay. Please don’t hesitate to reach out if you have any questions.

THE PHILOSOPHY OF LEAVING SOMETHING BEHIND WHEN YOU GO.

When you arrive at FLF you'll notice the centerpiece of our main group room contains a wide variety of different objects that have been left by our alumni. These include many challenge coins, patches, small tools of the trade, medals, and other mementos that are deeply meaningful to the people who have been through this program. Each day in our program group members are invited to choose a relic to be used as a "talking piece" which is passed from person to person during our Joining Forces group. We also encourage people to take a look through all of the relics at their leisure between groups and during down time. Some of these relics have been left to commemorate triumphs and successes. Some of them memorialize traumas and scars. All of them have been left by people just like you who understand the types of struggles and challenges you go through.

There is tremendous power and symbolism behind all of these objects and their associated stories. We gratefully encourage you to bring your own relic and add it to our collection during graduation on Friday. We will be honored to be stewards of the object you choose to leave whether it symbolizes a victory or whether it symbolizes some thought, feeling, image or memory that you're ready to leave behind.
